New Patients

If you are interested in becoming a new patient at UUANJ, please call us at (609) 581-5900 or (732) 928-5300 to schedule an appointment.

If your appointment was booked more than 2 weeks in advance, you should have received an envelope in the mail containing our new patient packet. If you did not receive anything in the mail from us we will give you a tablet computer to complete your patient registration upon your arrival, but ask that you arrive 30 minutes before your scheduled appointment time to fill out the paperwork.

Our office is fully electronic utilizing an electronic health record, so it is important that you fill out your patient history and review of systems with as much detail possible. Upon completion of your registration, our staff will attach your registration paperwork directly into your electronic chart. Please bring your insurance card and driver’s license with you to your appointment. These will also be scanned into our system. We will take a picture of you to place in your electronic record when you check-in, and collect your co-payment (if applicable).

 

The following is a listing of documentation that is crucial for our urologists to see prior to your office visit:

 If your insurance policy requires that you obtain a referral for specialist care, please obtain one prior to your appointment. If you have questions about this, please do not hesitate to contact our office.

  • Previous lab work mainly PSA values, urinalysis, or urine cultures you have had in the past. If you are referred for elevated PSA, your previous PSA’s are crucial in your treatment plan.
  • Recent radiology reports such as CT scans, MRI, or kidney ultrasounds. For any CT scan or MRI of the abdomen and/or pelvis, please bring a CD of the study to your appointment. Most of our doctors will review those images at the time of your appointment.
  • Any documentation from other doctors you have seen for your problem.
  • Current medication listing with correct spelling and dosage information. We ask that you bring the actual bottles to your visit to enable us to document the proper medication and dosage.
  • We do our best to stay on time with our clinic schedules. Emergencies, however, can and do occur and your physician may be delayed. We greatly appreciate your understanding in this matter.
